Are you an experienced clinical professional with a passion for system reform and patient-centred care?

We are seeking a Clinical Pathways Coordinator to join our State Control Centre – Clinical Hub in a permanent full-time role based in Belmont.

In this position, you will help shape the future of out-of-hospital care by designing, refining, and evaluating clinically appropriate patient pathways. Your work will play a key role in reducing emergency ambulance demand by facilitating timely, effective, and alternative care options in collaboration with both internal teams and external healthcare partners.

What you’ll do:

  • Lead the development and continuous improvement of clinical pathways aligned with strategic and patient safety standards.
  • Collaborate with internal teams and external healthcare partners to enhance patient outcomes and improve access to care.
  • Provide clinical oversight and support to the Clinical Hub and Secondary Triage teams.
  • Monitor the effectiveness and compliance of clinical decision support tools, contributing to audits and patient safety investigations.
  • Analyse performance data to drive continuous improvement and support strategic planning.
  • Support the onboarding, mentoring, and development of Clinical Hub staff.
  • Act as a key point of contact for the clinical team to facilitate, expand, and troubleshoot pathway delivery.
  • Build and maintain strong relationships with stakeholders to support robust care pathways that reduce emergency system demand (e.g., Urgent Care, Community Nursing, Aged Care respite, ECP, etc).

What you’ll need:

  • Registered health professional with current AHPRA registration
  • Postgraduate qualification in a relevant health-related discipline (highly desirable)
  • Extensive clinical experience in a jurisdictional ambulance service or primary or community healthcare setting
  • Demonstrated experience in clinical system reform, pathway development, and project management
  • Strong communication, leadership, and mentoring skills
  • Advanced skills in health stakeholder engagement and relationship management
  • Experience working with clinical decision support tools and interpreting complex health data
  • In-depth knowledge of WA’s healthcare system particularly relating to primary care, urgent care, and emergency services.
